Abstract
View PDFBackground: Juvenile Idiopathic Arthritis (JIA) poses profound clinical challenges in pediatric rheumatology, frequently necessitating prolonged and aggressive immunosuppressive therapy. In Ayurveda, the systemic and localized pathogenic mechanisms of JIA strongly correlate with Amavata, a debilitating systemic disorder characterized by the interplay of diminished digestive fire (Mandagni), toxic metabolic intermediates (Ama), and vitiated Vata localizing in the Sandhi (joints). Case Presentation: A 10-year-old female patient presented with a six-month history of symmetric polyarthritis, severe morning stiffness exceeding 90 minutes, extreme fatigue, and severely restricted ambulation. The clinical presentation and serological negativity aligned with Amavata (Polyarticular JIA). Intervention: A sequentially phased, inpatient Ayurvedic protocol was implemented over 30 days. It integrated oral Deepana-Pachana (metabolic correction) utilizing Amrutottaram Kashayam, localized Ruksha Sweda (dry heated sand bolus fomentation) to mitigate synovial effusion, and a modified Vaitarana Basti (therapeutic enema) schedule calibrated for pediatric tolerance to facilitate systemic Shodhana (purification). Outcomes: Following the intervention, the patient demonstrated profound clinical recovery. The Visual Analog Scale (VAS) for pain dropped from 8/10 to 2/10, morning stiffness reduced to less than 15 minutes, and serological inflammatory markers (ESR, CRP) returned to normal physiological baselines. Conclusion: The strategic application of Ruksha Sweda and Vaitarana Basti successfully interrupted disease progression, resolved systemic inflammation, and restored independent functional mobility. This case underscores the high efficacy and safety profile of classical Shodhana and Shamana modalities in managing pediatric Amavata, presenting a viable alternative to chronic immunosuppression.
